JeDi Diabetes Management Program

The Jefferson Diabetes Management Program offers personalized support, free supplies and medication, and expert guidance for nutrition, education and emotional well-being to help adult health plan members manage Type 1 and Type 2 diabetes. Prediabetes and gestational diabetes are not included.
