Care Management and Chronic Condition Support Services

Effective, July 1, 2026, Care Management services will be provided by Jefferson Care Solutions. These services include free, confidential support for serious, complex or hard-to-manage health conditions such as heart disease, high risk maternity, Post NICU, COPD, CKD, and any other complex medical conditions. These services are designed to connect you with coordinated care and personalized support to make managing your health a little easier.

Based on your health needs, you may also be eligible for the chronic condition support program. This includes one-on-one phone sessions with a licensed clinical health coach who can support you in a way that fits your lifestyle.
